Use the below form to request Equipment; to ensure accurate request you may only access this form while on campus.

Please let us know of your media technology requests by filling out this form, or call us at extension 5315.

 

NOTE: We need at LEAST 24 hours advance notice in order for your request to be honored. Requests are subject to equipment availability. If we cannot fulfill your request you will be notified.

Please be sure to submit the "official" Media Services Equipment Request Form so that we are aware of your media needs. If you are requesting equipment for an event, you may also contact the office of Conference & Scheduling. Thank you!
