




All undergraduate students must submit a Medical & Immunization History Form prior to the start of their first semester at Neumann University. Students enrolling for the fall semester must submit their Medical & Immunization History Form by August 1st. Students enrolling for the spring semester must submit their Medical & Immunization History Form by January 5th. It is recommended that graduate students also submit this form if they will be taking classes physically on the Neumann University campus. If your education is strictly online, this form is not required. Students who do not submit their Medical & Immunization History Form by the deadline may be restricted from moving into the residence halls or participating in classes until the form is submitted.
NEW STUDENTS: New students can submit their Medical & Immunization History Form through their application portal. Returning and graduate students can email Student Health Services to update their Medical & Immunization History Form anytime.
VACCINATION EXEMPTIONS: Neumann University follows the American College Health Association (ACHA), the Pennsylvania State Mandatory Vaccination Code, and the CDC’s immunization compliance guidelines. For the safety of all students, immunizations are mandatory. Per the Pennsylvania State Mandatory Vaccination Code, students may be exempted from the immunization requirements if a medical contraindication or the student’s religion prohibits immunization.
