Health Requirements & immunizations

All undergraduate students must submit a Medical & Immunization History Form prior to the start of their first semester at Neumann University. Students enrolling for the fall semester must submit their Medical & Immunization History Form by August 1st. Students enrolling for the spring semester must submit their Medical & Immunization History Form by January 5th.  It is recommended that graduate students also submit this form if they will be taking classes physically on the Neumann University campus. If your education is strictly online, this form is not required. Students who do not submit their Medical & Immunization History Form by the deadline may be restricted from moving into the residence halls or participating in classes until the form is submitted. 

NEW STUDENTS: New students can submit their Medical & Immunization History Form through their application portal. Returning and graduate students can email Student Health Services to update their Medical & Immunization History Form anytime.

VACCINATION EXEMPTIONS: Neumann University follows the American College Health Association (ACHA), the Pennsylvania State Mandatory Vaccination Code, and the CDC’s immunization compliance guidelines. For the safety of all students, immunizations are mandatory. Per the Pennsylvania State Mandatory Vaccination Code, students may be exempted from the immunization requirements if a medical contraindication or the student’s religion prohibits immunization. Immunizations

New Student Required Immunizations

  • Measles, Mumps, and Rubella (MMR): All newly enrolled or reenrolled students born after 1956 are required to show proof of (2) two MMR vaccinations. Learn more about MMR.
  • Tetanus/Diphtheria/Pertussis (Tdap): All newly enrolled or reenrolled students should have received the Tdap within 10 years. Learn more about Tdap.
  • Hepatitis B: (3) Three doses of Hepatitis B vaccine are needed for immunity. Doses one and two must be administered at least (4) four weeks apart. Dose three should be at least six months after the first dose and eight weeks after dose two. A blood test may be used to confirm immunity. Learn more about Hepatitis B.
  • Varicella: Two doses of live varicella vaccine or a physician documented history of the illness (chicken pox). Learn more about Chicken Pox.
  • Meningoccal Vaccine (Meningitis): The MCV4 Conjugate vaccine or MPSV4 Polysaccharide vaccines are acceptable. The brand names for the MCV4 Conjugate vaccines are Menactra, Menveo, Menquadfi, MenHibrix, or Penbraya. Learn more about Meningitis.

Recommended Immunizations

The following immunizations are strongly recommended (but not required) for all non-clinical Neumann University students.

  • Influenza Vaccination (Flu Shot): Recommended annually and available during the fall vaccination clinic. Learn more about Influenza. 
  • COVID-19 Vaccine: Recommended to stay current on the vaccination, including booster recommendations, which are available during the fall vaccination clinic. Learn more about COVID-19.  

Clinical Student Immunization Requirements

The following immunizations are required for clinical students in programs such as Nursing and Material Lab Science. Please note that these immunization requirements are subject to change. Faculty will provide additional information regarding documentation and immunization requirements before the start of clinical experiences.

  • Tetnus with Pertussis Booster: All clinical students are required to have received the Tdap within 10 years. Learn more about Tdap.
  • Measles, Mumps, Rubella: All clinical students are required to show proof of (2) two MMR vaccinations AND a documentation of positive titers. Learn more about MMR.
  • Hepatitis B Vaccination Series: All clinical students are required to provide a copy of their immunization records from all healthcare providers which includes the original series of Hepatitis vaccans and any boosters if possible. A titer showing antibodies consistent with immunity to Hepatitis B, drawn within 6 months of starting clinical rotations must be provided.  If lab work is not consistent with immunity, additional processes must be followed.   • Varicella (Chicken Pox) Vaccination: All clinical students are required to have two doses of live varicella vaccine or adocmentation of a positive titer. Learn more about Chicken Pox.
  • Tuberculosis (TB) Skin Test: All clinical students are required to have a 2-step PPD result skin test OR negative TB blood tests (QuantiFERON®-TB Gold OR T-SPOT). Learn more about Tuberculosis. 
  • Influence Vaccination: Required vaccination for clinical students to occur annually between August 31st and October 31st. Learn more about Influenza. 
Can I just email or drop off copies of my health history and immunizations from my doctor?
Students must complete the Medical & Immunization History Form through the link provided in their application portal. While medical forms from your doctor can help you fill out this form, please note that our form includes additional questions—such as emergency contact information—that may not appear on standard medical documents.

You’re also welcome to email or drop off a copy of your medical records Student Health Services in room 235 in Bachmann Main Building to supplement your submission. However, the official form must still be completed through the portal to ensure all required information is provided.

What is the due date for completing the Medical & Immunization History Form?

As an athlete, do I need to submit the Medical & Immunization History Form?
Yes. The Athletics Department uses a separate physical form specifically for student-athletes. However, information from the athletics physical can be used to help complete the electronic Medical & Immunization History Form available through the application portal.

Please note that new student-athletes are required to complete both forms—the athletics physical form and the university’s electronic health form—to ensure all necessary information is on file.
Do I need to complete the Medical & Immunization History Form as a commuter student?
Yes. All undergraduate students—whether living on campus or commuting—are required to complete the Medical & Immunization History Form.

This form is essential for maintaining a safe and healthy campus community. It ensures that Student Health Services has the necessary medical information to provide appropriate care and respond effectively in case of illness, injury, or emergency.
What is the process to request a medical or religious exemption of the required immunizations?
Neumann University permits exemptions to vaccination requirements for medical or religious reasons, provided proper documentation is submitted.

To request a medical exemption, students must provide a signed statement from a licensed healthcare provider—such as a Medical Doctor (MD), Doctor of Osteopathy (DO), Nurse Practitioner (NP), or Physician Assistant (PA). This statement must clearly explain which required vaccines are medically contraindicated and the specific medical reasons for each. Medical exemption documentation should be emailed to studenthealth@neumann.edu for review by the Student Health Services team.

For a religious exemption, students must submit a signed statement from a clergy member. The statement should describe the student’s sincerely held religious beliefs and explain how those beliefs prevent them from receiving the required vaccinations. Religious exemption letters should also be emailed to studenthealth@neumann.edu for review.

All exemption requests must be submitted by August 1 for the fall semester or January 5 for the spring semester in order to be considered.
